Policy Documents Overview

In some cases, your Blue Cross and Blue Shield of Montana policy document may be available without logging in to your secure Blue Access for Members℠ account or using the BCBSMT app.

Help locating your policy

  • Get your policy documents through your online account .
  • Get Your policy documents through the BCBSMT app. Learn more about the BCBSMT app. 
  • Contact a customer service representative at 1-800-447-7828 to get help finding or accessing your policy documents.
  • If you have coverage through an employer, you may want to contact the employer's benefit administrator to get the name of your plan.
  • If one of the following products or plan types sounds like your health care coverage, go to that page to find your plan and download its policy.

Small Group Policy Documents

  • You enrolled in a health plan through an employer,
  • The employer has between 1 and 50 employees, and
  • Your plan is a "metallic" (platinum, gold, silver or bronze) health plan.

Mid-Market Group Policy Documents

  • You enrolled in a health plan through an employer,
  • The employer has between 51 and 150 employees, 
  • Your have a fully-funded health plan  and
  • Your plan is not a "metallic" (platinum, gold, silver or bronze) health plan.

Blue Balance Funded Policy Documents

  • You enrolled in a health plan through an employer,
  • The employer has between 5 and 150 employees, 
  • Your have a self-funded health plan  and
  • Your plan is not a "metallic" (platinum, gold, silver or bronze) health plan.
