Tips For Completing the Form
- Independent Producers: You'll need your National Producer Number (NPN).
- Agencies: Those working on behalf of a licensed Agency may complete the Agency form. You'll need your Agency's NPN and your Agency Principal's NPN.
- Subproducers: Subproducers working for an agency do not contract with us directly so you don't need to complete a form. An agency contracts with us first. Then the agency works with us to "onboard" their subproducers.
