Getting Your Form 1095-B
You have four ways to ask for your BCBSMT Form 1095-B. Once we get your request, we'll mail it to you within 30 days.

Sign into your Blue Access for MembersSM account, then select "Forms and Documents" to download your 1095-B form.
Sign UpPhone
Call the customer service number listed on your BCBSMT member ID card to ask for your 1095-B form to be mailed to your address on file, or call 1-855-710-6984.
