You no longer need Form 1095-B to file your federal income taxes, but if you'd still like to use it, here's what you need to do.

Getting Your Form 1095-B

You have four ways to ask for your BCBSMT Form 1095-B. Once we get your request, we'll mail it to you within 30 days.

Online

Sign into your Blue Access for Members^SMaccount, then select "Forms and Documents" to download your 1095-B form.

Phone

Call the customer service number listed on your BCBSMT member ID card to ask for your 1095-B form to be mailed to your address on file, or call 1-855-710-6984.

Call Us

Mail

Fill out the 1095-B request form and mail it to the address listed on the form.

Print the form

If you bought your health plan on the Health Insurance Marketplace, you can visit the Marketplace website for details about your 1095-A form.
Based on your health plan, your employer may have sent you a 1095-C form.
